In a large, deep skillet or pot, heat the olive oil over medium-high heat. Add the chicken pieces and season with a pinch of salt and pepper. Sear until golden on all sides, about 4–5 minutes. Remove the chicken and set aside.
Lower the heat to medium. In the same pot, add the onion (if using) and red pepper. Sauté until softened and lightly caramelized, 3–4 minutes. Stir in the minced garlic and cook for about 30 seconds until fragrant.
Return the chicken to the pot. Sprinkle the Cajun seasoning over everything and stir to coat the chicken and vegetables evenly. Let the spices bloom for about 1 minute.
Add the uncooked pasta and chicken broth. Bring to a gentle boil, then reduce to a simmer. Cover and cook, stirring occasionally, until the pasta is al dente and most of the liquid is absorbed, about 10–12 minutes.
Stir in the heavy cream and grated Parmesan. Simmer until the sauce thickens and coats the pasta, 2–3 minutes. If using spinach or peas, add now and cook until just wilted or heated through. If the sauce is too thick, splash in a splash of broth or a little more cream until you reach your desired consistency.
Season to taste with salt, pepper, and optional red pepper flakes. Remove from heat, sprinkle with chopped parsley, and serve hot with extra Parmesan on the side.
